Patient Choice creates and manages patient-focused health care
networks. The company partners with other organizations to administer claims processing and customer service for its tiered network programs. These organizations include Medica (www.medica.com), UMR (https://member-fhs.umr.com)
and Aetna (www.aetna.com), among others.
If you have questions about your plan, including medical eligibility, benefits or claims, you should contact your program administrator. Contact information for your program administrator is listed on the back of your medical identification card.
If you are looking to enroll in a program that uses the Patient Choice Network and have not received a medical identification card, check with your employer for the appropriate contact information.
